What you get for each NPI

Each provider has a page with the full public record: legal name and credential, entity type, primary and secondary taxonomy codes with state license numbers, practice location and mailing address with phone and fax, other identifiers such as Medicaid IDs, the enumeration date and the date the record was last changed.

Organizations also show the authorized official, and any other names the business uses.

Why it is faster than the registry

The NPPES search page asks you to fill in a form for every lookup and shows results in pages of ten. Here you type a number or a name in one box. A number takes you straight to the provider, and a name lists up to 50 matches at once with specialty and city, so you can spot the right person quickly.

Records come from the same NPPES API, so you see what CMS has on file.

Questions

Is this NPI lookup free?
Yes. Searching by NPI number, name, city, state or specialty is free and needs no account. Only the bulk tool for checking more than 25 NPIs a day is paid.
Where does the data come from?
Every result is fetched from the official CMS NPPES NPI Registry API at the moment you search (cached for up to a day). Deactivated NPIs come from the CMS deactivated NPI report dated Nov 02 2026.
How do I look up an NPI number by name?
Type the first and last name in the search box, for example Maria Garcia. Add a state or city under the filters if the name is common. For an organization, type part of its legal business name.
How can I tell if an NPI is active?
Each provider page shows a status badge. Active records are in the NPPES registry. If an NPI has been deactivated, we show the deactivation date from the CMS report. See checking NPI status.
Can I look up many NPIs at once?
Yes. The bulk NPI lookup checks a list of up to 5,000 numbers and gives you a CSV with status, name, taxonomy, license and address for each.
Is this the official NPPES site?
No. npilookup.app is an independent site that reads the public NPPES data. Providers apply for and update an NPI on the official NPPES site.
